These are not equipment problems or process problems. Each one is a symptom of an organisational system that has not been designed for the complexity it is carrying.
Every significant decision — shift changes, maintenance priorities, cross-department coordination, quality disputes — reaches the plant head. The layer below is present but not functioning as an accountable management tier.
Production and maintenance do not coordinate reliably. Quality and operations operate with different priorities. Planning and execution are misaligned. At every functional boundary, friction accumulates and performance leaks.
Information transfer between shifts is informal. Issues discovered in one shift are not reliably communicated to the next. The operating system has no designed cross-shift accountability.

How well-defined are roles from plant head to supervisor? Are accountability boundaries explicit? Do role-holders understand what they are responsible for producing — not just what tasks they perform?

Which decisions are being made at which level? Where is decision-making being pushed upward unnecessarily? Are there structural gaps — decisions that no one at the operating level has authority to make?

How do production, maintenance, quality, safety, and planning coordinate at the site level? Where do handoffs fail? Are coordination mechanisms designed or dependent on personal relationships?

What meeting and review structures exist on the site? Are they at the right frequency and the right level? Do they produce decisions and course-corrections, or primarily generate data and reports?
